Increased Fracture Rate in Women With Breast Cancer: a Review of the Hidden Risk

Abstract (provisional)

Background

Women with breast cancer, particularly individuals diagnosed at a relatively early age, have an increased incidence of fractures. Fractures can have serious clinical consequences including the need for major surgery, increased morbidity and mortality, increased cost of disease management, and reduced quality of life for patients. The primary cause of the increased fracture risk appears to be an accelerated decrease in bone mineral density (BMD) resulting from the loss of estrogenic signaling that occurs with most treatments for breast cancer, including aromatase inhibitors. However, factors other than BMD levels alone may influence treatment decisions to reduce fracture risk in this setting. Our purpose is to review current evidence for BMD loss and fracture risk during treatment for breast cancer and discuss pharmacologic means to reduce this risk.

Results

Fracture risk during treatment for breast cancer may be influenced by the rate of BMD loss and the consequent rapid alterations in bone microarchitecture, in addition to the established fracture risk factors in postmenopausal osteoporosis. The rapid decrease in BMD during adjuvant chemoendocrine therapy for breast cancer may necessitate more aggressive pharmacotherapy than is indicated for healthy postmenopausal women who develop osteoporosis. Over the last few years, clinical trials have established the effectiveness of bisphosphonates and other antiresorptive agents to preserve BMD during adjuvant therapy for early breast cancer. In addition, some bisphosphonates (eg, zoledronic acid) may also delay disease recurrence in women with hormone-responsive tumors, thereby providing an adjuvant benefit in addition to preserving BMD and potentially preventing fractures.

Conclusions

It is likely that a combined fracture risk assessment (eg, as in the WHO FRAX algorithm) will more accurately identify both women with postmenopausal osteoporosis and women with breast cancer who require bone-protective therapy.

Deployment of unified communications solutions is poised for growth as organizations build on a foundation of voice and data applications to include more video, collaboration and social communications tools, according to new research from CompTIA, the non-profit association for the information technology (IT) industry.

Nearly half (49 percent) of the organizations surveyed for the new CompTIA study, Unified Communications and Collaboration Market Trends, said their expenditures on unified communications technologies will grow relatively faster than their overall IT budget over the next 12 months.

Large firms (500 or more employees) are significantly more likely to increase their unified communications investment relative to the overall IT budget than the smallest of firms (1-49 employees), 64 percent vs. 35 percent.

?This likely reflects the complexity of communications at a large firm compared to a small firm,? said Tim Herbert, vice president, research, CompTIA. ?More staff, more locations, more end-points and possibly more IT systems make for a more complex communications landscape and a stronger desire to simplify through a unified communications strategy.?

IT channel companies express similar positive sentiments about growth in unified communications adoption. Among IT firms with a unified communications practice, 31 percent expect significant growth in their practice over the next 12 months, while 59 percent expect modest growth. Few expect a drop-off in their unified communications business.

While IT companies and their customers are bullish on the future of unified communications, the CompTIA study indicates that greater clarity about what constitutes unified communications is needed.

From the IT channel perspective, technology product and solution providers also have several hurdles to overcome with customers. These customer challenges include price sensitivity, cited by 39 percent of channel respondents; reliability concerns (36 percent); security concerns (34 percent); difficulty in quantifying return on investment (33 percent); and a general lack of understanding of unified communications products and services (32 percent)

Defining Unified Communications

Customers and their technology partners are fairly consistent when asked to define unified communications. For each group, core areas include e-mail, web conferencing, unified messaging, video conferencing, audio conferencing and IP communications.

But despite the media attention of technologies such as social communication and location-based services, they are not yet strongly associated with unified communications, according to the CompTIA study.

Additionally, fewer respondents have made the leap from viewing unified communications as an incremental improvement for interaction and sharing to the higher-level communications-enabled business processes. This is seen in the relatively lower numbers of respondents making a strong connection between unified communications with other enterprise systems such as customer relationship management tools.

Many of the technologies associated with unified communications are already widely adopted, according to the CompTIA study. For example, 64 percent of organizations surveyed have Web conferencing; 58 percent, use video conferencing; 54 percent, collaboration applications or platforms; and 51 percent, voice-over-IP (VoIP) telephony.

?The integration of all these elements is the hard part, tying all these things together,? said Seth Robinson, director, technology analysis, CompTIA. ?Voice and data will still be important, but more effort will be devoted to complement them by bringing more video, collaboration and social elements into the enterprise.?

Before this can happen, however, organizations must perform a network analysis to determine if their network can support the requirements for new solutions.

?Voice and video are the components of a solution that will drive network upgrades since they consume the most bandwidth and must be handled properly to assure high quality,? Robinson explained.

Indeed, among companies in the CompTIA study who have installed a VoIP solution, 61 percent upgraded network equipment such as routers and switches; and 51 percent upgraded infrastructure, such as cabling and network drops.

CompTIA?s Unified Communications and Collaboration Market Trends study is based on separate online surveys of 600 IT and business executives involved in their organization?s communications initiatives or strategies and 300 companies in the IT channel. We see it in the movies and on television when a character realizes they desperately want to have a child. Often it is connected with a ticking biological clock. Or we may experience it ourselves when we see baby toys and clothes in the store. "It" can be summarized in two words: Baby fever.

Not only does the phenomena called baby fever exist, it is found in both men and women, according to researchers from Kansas State University. Gary Brase, associate professor of psychology, and his wife, Sandra Brase, a project coordinator with the university's College of Education, have spent nearly 10 years researching baby fever: the physical and emotional desire to have a baby.

"Baby fever is this idea out in popular media that at some point in their lives, people get this sudden change in their desire to have children," Gary Brase said. "While it is often portrayed in women, we noticed it in men, too."

The Brases' interest in studying baby fever started shortly after the birth of their second child.

"I noticed a distinct difference in my desire to have more children," Sandra Brase said. "Although one hears about people having baby fever from friends, family and in the media, I was curious if there was a scientific explanation for the presence or lack of it in both women and men."

While some research has looked at the demographic and sociological aspects of having children, there had been no previous study from a psychological perspective, Sandra Brase said. The Kansas State University research appears in the upcoming issue of Emotion, which is published by the American Psychological Association.

The researchers started by applying three different theoretical viewpoints about why baby fever might exist and where it came from:

The sociocultural view: People want to have a baby because they are taught gender roles. Women think they should have children because society says that is what they are supposed to do.
The byproduct view: Humans experience nurturance. When they see a cute baby they want to take care of it, and that makes them want to have a baby of their own. Baby fever is a by-product -- it is nurturance misplaced.
The adaptationist view: Baby fever is an emotional signal -- like a suggestion sent from one part of the mind to the other parts -- that this this could be a good time to have a child.

The researchers then performed studies to understand people's desires, particularly the desire to have a baby.

"Sometimes you may have a desire to have a baby, sometimes you have desires to have money or be famous or have sex," said Gary Brase, whose research focuses on judgment and decision-making. "We asked people to tell us where these desires ranked."

The researchers found several interesting results: First, that baby fever did exist and it existed in both genders. But how frequently a desire for a baby occurred varied according to gender. Women more frequently desired having a child than having sex. Men were the opposite and more frequently desired sex than having a child.

"We found this kind of ironic because sex and having a baby are causally related," Gary Brase said.

The researchers also asked people to describe what led them to want and not want to have a baby. Based on these responses, the researchers created a questionnaire that asked participants their attitudes toward children and fertility. They surveyed college students and also used an online survey to reach a wider range of participants.

"The idea that gender role or misplaced nurturance are the major driving forces didn't get a lot of support from our study," Gary Brase said. "It is something much more fundamental than that."

Rather, the researchers found three factors that consistently predicted how much a person wanted to have a baby. The first factor was positive exposure -- such as holding and cuddling babies, looking after babies and looking at baby clothes and toys -- that made people want to have a baby. The second factor included negative exposure -- such as babies crying, children having tantrums and diapers, spit-up or other 'disgusting' aspects of babies -- that made people not want to have a baby. The third factor included trade-offs that come with having children -- education, career, money and social life.

"We had people who were high on the positive aspects and they see all the good things about babies and want a baby," Gary Brase said. "We also had people who were high on the negative aspects and absolutely do not want to have babies. Then we had people who were high on both positive and negative aspects and were very conflicted about children.

"Having children is kind of the reason we exist -- to reproduce and pass our genes on to the next generation," he said. "But economically, having children is expensive and you don't get any decent financial return on this investment. And yet, here we are, actual people kind of stuck in the middle."

The researchers plan follow-up studies that focus on the role of hormones and why people might experience high and low levels of baby fever.

WARSAW, Poland ? A Polish judge has found a death metal singer innocent of offending religious feelings, ruling that his ripping up of a Bible during a show was a form of artistic expression consistent with the style of his band.

Adam Darski, who goes by the stage name Nergal and is the frontman for the death metal band Behemoth, was charged after he ripped up the Bible during a 2007 concert in Gdynia.

He was found innocent by a court last year but prosecutors appealed that verdict. The court issued an innocent verdict Thursday.

Darski threw the torn pages to the audience during the concert, and asked fans to burn them, according to video footage from that concert. According to the Polish news agency PAP, he also called the Bible "a deceitful book" and the church a "criminal sect" ? strong language in this predominantly Roman Catholic country.

Judge Krzysztof Wieckowski, in issuing his ruling, said it considered Darski's actions "a form of art" consistent with the style of his band. He added that the court had no intention of limiting freedom of expression or the right to criticize religion.

The court said audience members who testified said their religious feelings had not been hurt despite the fact they were Christians.

Offending another person's religious feelings is a criminal offense in Poland, and can carry a prison sentence of up to two years.

SAN FRANCISCO (Reuters) ? The hacker group Anonymous is taking on San Francisco's subway system after the Bay Area Rapid Transit turned off cell phone service in stations last week to foil a different protest.

The transit service last Thursday shut down cell phone networks in some stations to stop a protest over a police shooting.

In a city known for civil rights protests and technology, that tactic has landed the commuter train system in a new controversy.

"We've invited these protesters now," said Lynette Sweet, a BART director who said managers of the system should not have announced the in-station mobile phone network shutdown.

Anonymous is a loosely knit group that has attacked financial and government Web sites, frequently claiming civil rights motives.

They called for protesters to descend on the Civic Center station in San Francisco at 5 p.m. on Monday, publicizing the action with tweets from @YourAnonNews.

Would-be protesters were encouraged to download software for short-range mobile-to-mobile messaging, in case the in-station networks are shut down again.

BART said that a Web site for its users, myBART.org, had been hacked over the weekend, and that contact information from at least 2,400 people had been stolen.

SAN FRANCISCO (AP) ? San Francisco's mass transit system prepared for renewed protests Monday, a day after hackers angry over blocked cell phone service at some transit stations broke into a website and posted company contact information for more than 2,000 customers.

The action by a hacker group known as Anonymous was the latest showdown between anarchists angry at perceived attempts to limit free speech and officials trying to control protests that grow out of social networking and have the potential to become violent.

Anonymous posted people's names, phone numbers, and street and email addresses on its own website, while also calling for a disruption of the Bay Area Rapid Transit's evening commute Monday.

BART officials said Sunday that they were working a strategy to try to block any efforts by protesters to try to disrupt the service.

"We have been planning for the protests that are said to be shaping up for tomorrow," BART spokesman Jim Allison said. He did not provide specifics, but said BART police will be staffing stations and trains and that the agency had already contacted San Francisco police.

The transit agency disabled the effected website, myBART.org, Sunday night after it also had been altered by apparent hackers who posted images of the so-called Guy Fawkes masks that anarchists have previously worn when showing up to physical protests.

The cyber attack came in response to the BART's decision to block wireless service in several of its San Francisco stations Thursday night as the agency aimed to thwart a planned protest over a transit police shooting. Officials said the protest had been designed to disrupt the evening commute.

"We are Anonymous, we are your citizens, we are the people, we do not tolerate oppression from any government agency," the hackers wrote on their own website. "BART has proved multiple times that they have no problem exploiting and abusing the people."

Allison described myBART.org as a "satellite site" used for marketing purposes. It's operated by an outside company and sends BART alerts and other information to customers, Allison said.

The names and contact info published by Sunday came from a database of 55,000 subscribers, he said. He did not know if the group had obtained information from all the subscribers, he said, adding that no bank account or credit card information was listed.

The BART computer problem was the latest hack the loosely organized group claimed credit for this year. Last month, the FBI and British and Dutch officials made 21 arrests, many of them related to the group's attacks on Internet payment provider PayPal Inc., which has been targeted over its refusal to process donations to WikiLeaks. The group also claims credit for disrupting the websites of Visa and MasterCard in December when the credit card companies stopped processing donations to WikiLeaks and its founder, Julian Assange.

BART's decision to shut down wireless access was criticized by many as heavy handed, and some raised questions about whether the move violated free speech.

The problems began Thursday night when BART officials blocked wireless access to disrupt organization of a demonstration protesting the July 3 shooting death by BART police who said the 45-year-old victim was wielding a knife.

Activists also remain upset by the 2009 death of Oscar Grant, an unarmed black passenger who was shot by a white officer on an Oakland train platform. The officer quit the force and was convicted of involuntary manslaughter after the shooting.

Facing backlash from civil rights advocates and one of its own board members, BART has defended the decision to block cell phone use, with Allison saying the cell phone disruptions were legal because the agency owns the property and infrastructure.

"I'm just shocked that they didn't think about the implications of this. We really don't have the right to be this type of censor," Lynette Sweet, who serves on BART's board of directors, said previously. "In my opinion, we've let the actions of a few people affect everybody. And that's not fair."

Laura Eichman was among those whose email and home phone number were published by the hackers Sunday.

"I think what they (the hackers) did was illegal and wrong. I work in IT myself, and I think that this was not ethical hacking. I think this was completely unjustified," Eichman said.

She said she doesn't blame BART and feels its action earlier in the week of blocking cell phone service was reasonable.

"It doesn't necessarily keep me from taking BART in the future but I will certainly have to review where I set up accounts and what kind of data I'm going to keep online," Eichman said.

Michael Beekman of San Francisco told the AP that he didn't approve of BART's move to cut cell phone service or the Anonymous posting.

"I'm not paranoid but i feel like it was an invasion of privacy," he said. "I thought I would never personally be involved in any of their (Anonymous') shenanigans."

The group Anonymous, according to its website, does "not tolerate oppression from any government agency," and it said it was releasing the User Info Database of MyBart.gov as one of many actions to come.

"We apologize to any citizen that has his information published, but you should go to BART and ask them why your information wasn't secure with them," the statement said.
